Right between your shoulder blades sits the most neglected stretch of your spine — twelve vertebrae that were built to twist, extend, and rotate, yet in most adults barely move at all. When this region goes quiet, your neck and lower back pick up the slack, and that's usually where the pain shows up. Today, let's give the middle back the attention it deserves.

Meet Your Thoracic Spine

Your spine has three main regions: the cervical spine (neck), the lumbar spine (lower back), and between them, the thoracic spine — twelve vertebrae running from the base of your neck to the bottom of your rib cage. Each one connects to a pair of ribs, which makes this region wonderfully stable. It protects your heart and lungs, anchors your posture, and serves as the engine room for rotation — the twisting motion you use every time you swing a golf club, reach into the back seat, or look over your shoulder.

That stability comes with a catch. Because the thoracic spine can get away with not moving, in modern life it usually doesn't. The Borrowing Problem: Why a Stiff Middle Back Hurts Somewhere Else

Here's the part that surprises most of my patients: a stiff thoracic spine rarely hurts in the thoracic spine. Instead, the pain shows up above and below it.

Your body is a master of compensation. Movement has to come from somewhere, so when the twelve segments in the middle stop rotating and extending, your body "borrows" that motion from the nearest available source — the neck above and the lower back below. The trouble is that those regions weren't designed for the extra workload. The lumbar spine is built primarily for bending forward and back, not for twisting. The neck is built for fine, precise movement, not for hauling your whole upper body around.

Over months and years, that borrowed motion adds up to familiar complaints:

• Neck pain and tension headaches — when the mid-back won't extend, the head juts forward and the small muscles at the base of the skull work overtime.
• Lower back strain — twisting motions (golf, tennis, shoveling, even housework) get forced through lumbar joints that were never meant to rotate far.

The Breathing Connection

Because your ribs hinge on your thoracic vertebrae, mobility here is quite literally the difference between a full breath and a partial one. A flexible rib cage expands in three dimensions — front, sides, and back — letting the diaphragm descend fully. A stiff one forces you into shallow, upper-chest breathing, which recruits neck muscles as accessory breathers and feeds the very tension you're trying to escape.

This is one of my favorite examples of how the body works as a system: improve the mobility of twelve joints in your mid-back, and you can improve your breathing, your neck tension, and your stress response all at once.

A 30-Second Self-Test

Try this: sit tall on a chair with your knees together and cross your arms over your chest. Keeping your hips square and knees still, rotate your shoulders to the right as far as you comfortably can, then to the left. You should be able to turn roughly 45 degrees in each direction — enough that your shoulders approach the diagonal of the chair. If you're well short of that, or one side turns noticeably less than the other, your thoracic spine is asking for attention.

Your Daily Middle-Back Mobility Routine

The good news: the thoracic spine responds quickly to consistent, gentle movement. Five minutes a day beats an hour once a week. Here's a simple routine you can do at home:

• 1. Foam Roller Extensions — 1 minute — Lie back over a foam roller placed across your mid-back, hands supporting your head, knees bent. Gently arch backward over the roller, exhale, and let the segment open. Move the roller up an inch and repeat, working between the bottom of the shoulder blades and the base of the neck. Avoid arching through the lower back.
• 2. Open Books — 8 per side — Lie on your side with knees stacked and bent to 90 degrees, arms extended together in front of you. Keeping the knees glued together, sweep your top arm up and across to the opposite side like a book opening, following your hand with your eyes. Exhale as you open. Feel the rotation come from the rib cage, not the arm.
• 3. Thread the Needle — 6 per side — From hands and knees, slide one arm under your body, palm up, letting your shoulder and ear move toward the floor — then unwind and reach that arm up toward the ceiling. This pairs rotation with the gentle traction the mid-back loves.
• 4. Wall Angels — 10 slow reps — Stand with your back against a wall, arms in a goalpost position. Slowly slide your arms up and down, keeping forearms and lower back as close to the wall as you can. This trains extension and shoulder blade control together.
• 5. Walk With Arm Swing — daily — A brisk walk with relaxed, natural arm swing gently rotates the thoracic spine a thousand times per mile. It's the most underrated mobility tool you own.
• Desk worker tip: set a reminder every 45 minutes to stand, place your hands behind your head, and gently extend backward over the top of your chair for three slow breaths. Ten seconds, several times a day, keeps the stiffness from accumulating in the first place.

Where Chiropractic Care Fits In

Sometimes a thoracic segment isn't just tight — it's genuinely restricted, and no amount of foam rolling will coax it to move. This is where specific chiropractic adjustment earns its keep.
